~ First Visit ~

Please print, complete, initial and sign these two forms prior to your first visit.  Bring them with you to your appointment.

If you have a pain condition, please print and complete this Pain Questionnaire, and bring it with you to your appointment.


Here are Directions and Parking Information to my office.  

When you arrive, please park on either side of the double driveway, leaving room for another vehicle beside you.

 

Once you have parked, please text or call me: I will escort you inside.  Please allow up to 5 minutes for me to come out to greet you before ringing the doorbell.

 

Bring a fresh mask, hand sanitizer, your own pen and water bottle to your appointment.  

 

Wear or bring a change of clothing that is loose, casual and comfortable.  Stiff or tight jeans, belts, bulky clothes and tight sleeves and pant cuffs all interfere with the treatment.  

 

Please remove all jewelry and wrist watches, electronic devices, loose change, etc. from your pockets prior to treatment.  Then, apply hand sanitizer.

 

Please do not wear perfume out of consideration for those with respiratory allergies.

 

Please notify me before your appointment if you have shortness of breath, fever, cough, runny nose, sore throat, loss of taste or smell, or you feel like you might be getting sick.  Please notify me if you have travelled in the past 2 weeks, attended any mass events, or come into contact with a Covid-19 patient or with anyone exhibiting signs of acute respiratory infection.

 

It is best to arrive for your appointment not too hungry and not too full.  Eating a light meal or snack before your appointment is fine. 

 

I have a 24-hour cancellation policy.  If you need to cancel or change your appointment time, notify me at 617-964-9519 at least 24 hours beforehand.  There is no cancellation charge if you feel like you may be developing a cold or flu, or if you’ve been exposed to a sick or coughing person since you scheduled your appointment.
